    Patient Portal

    FollowMyHealth is now our patient portal of choice to provide you with access to your medical information to help you manage your health.

    What this means for you:
    Historical medical information will continue to be available in your RelayHealth patient portal.  However, no new updates will be made to this account.  All new items will be sent to your FollowMyHealth account.   

    Independent physician practices utilize a different patient portal and will NOT be affected by this change.

    What you need to do:
    RelayHealth users will need to register for FollowMyHealth to establish access to your new account. You will automatically receive an email inviting you to register through a secure personal link from noreply@followmyhealth.com. (Be sure to add this to your safe list!)

    FollowMyHealth will not automatically pull your historical information from RelayHealth. Though, you can easily add or import data into FollowMyHealth.

    One of the many great features of FollowMyHealth is that you can download their mobile app. This will give you easy access on-the-go, making sure you always have your information at hand.

         		 Provide a valid e-mail address at registration during your visit 
    to receive a personalized invitation via e-mail. 
    Follow the link to verify your identity and create your account.

    To access your
    medical record on
    your mobile device         		 Download the Medlio app from the Google Play app store or from Apple’s App Store.
    Once this is completed and you request your health related info from Medlio, your identity will be authenticated using your secure FollowMyHealth account.

    We take great care to ensure your health information is kept private and secure by adhering to the latest security standards and utilizing advanced encryption technology. Access to information is controlled through a secure invitation process, personal IDs and passwords.

    Your name and e-mail address will be treated with the same care and privacy given your health records, and will never be sold or leased by Rome Memorial Hospital or our Patient Portal partners.

    Your Personal Health Record

    The information provided in the Patient Portal is for your personal use. You may enter additional information; however, it will not be reflected in your medical record maintained by the hospital. Any questions regarding test results should be directed to your physician.

    Physician Office Patient Portals

    Many physician offices have or are in the process of launching their own patient portals. Your physician’s portal will require a separate registration and log-in.
